This entails understanding why a person seeks medical attention. That forms the basis of a provisional diagnosis and relevant investigations. Provided the investigations, and patient’s symptoms match, if that can be reliably improved by a surgical procedure; Dr. Shad would advise surgery.
Along with the advice of surgery, he thoroughly explains what benefits might accrue from heart surgery and why he prefers surgery for you and not medical/ non-surgical treatments. Since each surgical procedure carries a defined and finite risk, he gives you a numbered risk. For example, a risk of death and major complications of 0.6% for routine bypass surgery is usual.
Sometimes, surgery would be conducted emergently for conditions that shouldn’t wait for even a few hours like for aortic dissection or aortic valve disruption by infection. Sometimes a very sick patient is first admitted for around a week to improve the patient’s condition before surgery.
All this would be informed in clear Hindi, English, or Punjabi to the family and patient sitting together. He doesn’t believe in ‘please don’t tell the patient that he/she is to undergo surgery’.
